Launching a new care program can sound like a heavy lift. In practice, it’s a short, structured process — and we carry most of the work.
Step 1: Discovery Call
We start with a conversation about your patient population, current workflows, and which programs make sense for your practice — whether that’s RPM for your hypertension and diabetes patients, CCM for general chronic care coordination, or a combination of programs.
Step 2: Program Design & Setup
We map out enrollment criteria, care plan templates, device selection, and documentation workflows tailored to your practice. If you use an EHR, we coordinate how program data and notes fit into your existing system.
Step 3: Staff Orientation & Pilot
Your team gets a short orientation on how the program works day to day — what to expect, who to contact, and how billing documentation comes together. We typically start with a small pilot group of patients to validate the workflow before rolling out further.
Step 4: Full Enrollment & Ongoing Support
Once the pilot is running smoothly, we scale enrollment across your eligible patient population. From there, we provide ongoing monitoring support, regular reporting, and check-ins to keep the program running well as it grows
We’re based in the San Francisco Bay Area, with immediate focus on clinics and pharmacies. That means real, local support.
