What does this cost my practice?
There’s no upfront software fee for your practice. These are Medicare-reimbursable programs — practices bill Medicare, and many commercial payers, monthly per enrolled patient under standard billing codes. We’ll walk you through realistic reimbursement ranges for your patient mix before you commit to anything.
How much staff time does this take?
Very little of your team’s time is required to launch. We handle patient onboarding, device logistics, consent, documentation, and day-to-day monitoring support. Your clinical staff stays in the loop and retains full clinical authority — we carry the operational workload.
How long does it take to launch a program?
Most practices are actively enrolling patients within a few weeks of getting started. We begin with a short discovery conversation, then move through setup, staff orientation, and a small pilot group before scaling to your full eligible population.
Do our patients need Wi-Fi, an app, or a smartphone?
No. The devices we use are cellular-connected — they transmit readings automatically, with no app, pairing, or home internet required. This matters a lot for older patients and patients who aren’t comfortable with technology.
Is this HIPAA compliant?
Yes. Every part of the program — data transmission, storage, and our internal processes — follows HIPAA requirements, and we execute a Business Associate Agreement with every practice we work with. See our Compliance page for details. We already work with a monitoring or care management vendor.
Can you help anyway?
Often, yes. We can evaluate what you’re currently using and either improve the workflow around it or help you transition to a better-fitting setup — whichever makes more sense for your practice.
Which patients qualify?
Eligibility depends on the specific program, but generally includes patients with one or more chronic conditions, a recent hospital discharge, or a need for closer medication or behavioral health follow-up. We help identify eligible patients within your existing panel as part of setup.
Who bills for the service, and whose name is it under?
All billing runs under your practice’s name and provider NPI. You retain full clinical and billing ownership — we provide the operational and administrative support behind the scenes.
Do we need to buy any equipment or software?
No purchase is required from your practice. Devices are shipped directly to enrolled patients, and the monitoring platform is included as part of the program.
What’s the difference between working with you and going directly to a software vendor?
A software platform gives you a dashboard — it doesn’t onboard your patients, chase down devices, staff a monitoring team, or manage your documentation. We handle the entire operational layer so the program actually runs, not just exists.
